Cultivator Disclosure Statement
The National Institute of Health categorizes cannabis is a Complementary and Alternative Medicine (CAM). California SB 577 Regulations recognize the individuals who provide CAM health care services and products as CAM Practitioners.
The SB 577 Regulations require CAM Practitioners to provide a “Disclosure Statement” regarding their services and qualifications. The Disclosure Statement is signed by the Patient.
The CMCC provides a boilerplate copy of a compliant CAM Practitioner Disclosure Statement.
Protection of patient’s ID information and Physician Approval
The patient’s ID information and the CMCC verified Physician Approval are stored in the CMCC’s HIPAA compliant patient medical records system.
The patient’s contact information is the only information forwarded to the cultivator.

Product Testing
The CMCC Product Testing Committee is responsible for random batch testing of products offered on the CannabisMED.
Random, batch testing, is the way the Department of Cannabis Control tests for the safety of products sold by city and state licensed cannabis retail facilities.
Reasonable Compensation
Transactions are directly between the qualified patient and the cultivator. With the elimination of the middleman, the cultivator’s reasonable compensation is intended to help medical cannabis patients save money.
Sales Taxes
Cultivators are required to have a Sellers Permit from the State Board of Equalization.
Patients who have a Department of Public Health (DPH) Medical Marijuana Patient ID Card are exempt from paying Sales Taxes.
Grow Site Compliance
Cultivators agree to cooperate with the CMCC Compliance Advisor to ensure compliance with SEC. 137. Section 11362.765 of the Health and Safety Code.
Product Orders and Sales
All transactions are directly between the qualified Patient (or Patient’s Primary Caregiver) and the cultivator.
Delivery Services
It is the cultivator’s responsibility to deliver the product to the buyer.
Complaints/Report Issues
The website Menu includes a “Report an Issue” button that may be used by qualified patients and cultivators to report an issue or complaint.
The CMCC Patient Services Committee is responsible for documenting and responding to a reported issue.
The CMCC is a member of the Better Business Bureau and follows the BBB guidelines for processing and resolving a complaint.
CMCC Monthly Forum
The CMCC Forum is held on-line on the first Monday of the month at 12:00 pm via Zoom. The meeting agenda includes CMCC Committee reports and recommendations from participating cultivators.
